To ensure your smooth transition from summer rest and recreation to the fall enrollment process at Manhattan School of Music, please use this checklist to reconcile any outstanding matters prior to arriving at Manhattan School of Music in August.

All final documents listed in this section must be received by the Office of Admission no later than August 4, 2010. If all required documents are not received by this date, you will not be able to register for the Fall 2010 semester. In addition, International students' visas may be adversely affected. A late fee of $100.00 U.S. will be assessed to anyone whose file is not complete by the date listed above, and you will not be able to register until all required materials are received AND THE LATE FEE IS PAID. TRANSCRIPTS
Final, official, and original transcripts from ALL previous institutions attended. Notice: Copies and faxes of transcripts are not considered official.

PROOF OF DEGREE OR DIPLOMA
Official proof of degree or diploma is required for all FRESHMAN, GRADUATE, and DOCTORAL students. Many U.S. transcripts will list a date of graduation or a statement and date that a diploma/degree has been earned or conferred.

IMMUNIZATION RECORDS
The Immunization History Form must be completed and signed by your physician or appropriate medical personnel. The Meningitis Immunity Form must be completed and signed by the student or the student's guardian.

SUMMER CONTACT INFORMATION
Please keep Manhattan School of Music updated on your whereabouts over the summer. The Office of Admission may need to be in contact with you before your arrival this fall.

TRANSLATIONS and WES
INTERNATIONAL students and any STUDENTS WHO HAVE STUDIED ABROAD must provide an official, notarized translation of a non-English transcript. Any student who was asked to provide a World Education Services (WES) Evaluation (www.wes.org) must request that WES send an official and original report directly to Manhattan School of Music (electronic delivery preferred). Note: Copies and faxes of translations are not considered official.

Award letters must be SIGNED and returned by June 15 , 2010.

Complete a Master Promissory Note online at http://www.dlenote.gov by July 15, 2010.

ENTRANCE LOAN COUNSELING
All borrowers must complete Entrance Loan Counseling. Please come back for the link in mid-April.

REVIEW YOUR INVOICE AND FINANCIAL AID PACKAGE


DETERMINE THE BALANCE NOT PAID BY FINANCIAL AID.
You, the student, are the account holder and fully responsible for paying the balance on or before the due dates regardless of the source of funds.

LATE FEES AND OTHER CONSQUENCES
Understand that late fees and other consequences will apply to ALL accounts not paid by the due date as follows:
  • August 9, 2010 – $150
  • August 23, 2010 – $250
  • September 7, 2010 – $350
  • September 21, 2010 – Possible suspension from all School activities, including major lessons
PAYMENT OPTIONS.
Review payment options as described in the enclosed Tuition and Fees brochure and select the one that fits your financial situation.
  • Pay in Full – Total balance (net of expected financial aid) is due July 27
  • MSM Deferred Payment Plan (DPP) – Half of total balance due (net of expected financial aid) PLUS $100 DPP fee is due July 27; Second half is due October 1
  • Sallie Mae TuitionPay Plan – See enclosed enrollment form. You will submit your payments directly to Sallie Mae.

STUDENT AUTHORIZATION
Sign and return STUDENT AUTHORIZATION TO RELEASE EDUCATION RECORDS TO PARENT(S)/GUARDIAN(S)/SPONSOR(S) form. Staff will NOT discuss your student account with anyone except YOU, the student, and individuals indicated in writing on this form.

Please be sure to submit your completed I-20 application as soon as possible.

If the I-20 application is incomplete, your I-20 cannot be issued to you. As the processing of an I-20 application can take anywhere from 4 to 6 weeks, please be sure to submit all required materials as soon as possible.

ID Cards, Health Insurance, and Emergency Contact Form

All Manhattan School of Music students need to complete steps to make sure they have a Manhattan School of Music ID Card, heath insurance that can be used in NYC, and an emergency form. The following link contains information to guide you to successfully take care of each of these items.



MSM Identification Card (digital photo is needed by July 30, 2010)

All MSM students are required to carry a Manhattan School of music identification card at all times. This card facilitates student's ability to access MSM buildings, meal plan, library privileges, and practice rooms. All new students must email a digital photo Office of Information Technology by July 30, 2010. The photo should be taken at 72 DPI resolution and be taken in portrait orientatin (ideally the photo should be 300 pixels wide and 375 pixels high. If you are unaware of the pixels or resolution, simply send us one of yourself that is a clear and close up photo of your face, with a neutral background. Do not wear sunglasses or hats. Please email the photo with your name and birth date to studentphoto@msmnyc.edu.

Manhattan School of Music has the right to reject any photo and to request a new photo. Those who do not supply an acceptable photo by the deadline will need to be extremely patient as they check in as their id card will not be ready upon arrival.



Emergency Contact Form

Use this form to list all people who should be contacted in an emergency. All students must complete a new Emergency Contact Form each year. This form is due immediately, preferably by May 14, 2010 for optimal results, and before June 30, 2010. This form is also available in the Housing section for residents.

Health Insurance

Health insurance is required of all full time students at Manhattan School of Music. The health insurance should be easily used in New York City to take care of any sickness or accident that may occur. Manhattan School of Music provides a health insurance plan at a reasonable cost, but allows those who are covered by equitable insurance to waive out of the insurance. Please complete the Health Insurance Information Form and submit it to the Office of Student and Residence Life by May 20, 2010. Students with alternate insurance must submit the form with a copy of their insurance card, front and back.

Orientation

All new Manhattan School of Music graduate and undergraduate students are required to check in to the School on Wednesday, August 25, 2010 between 8am and 1pm. Events to orient students to Manhattan School of Music, New York City, their departments, and their new colleagues will occur from August 25, 2010 through the start of classes. On August 25, 2010 there will be sessions for parents and guardians too. It is essential that new students participate in Orientation as placement exams and placement auditions will take place during this time.

 

Housing

Andersen Hall is an amazing living learning community for musicians! Manhattan School of Music encourages all students to live in Andersen Hall where they will have an opportunity to bond with and build a community with other musicians. Please note that there is a two-year housing requirement for all new students. The policy states:

"All first- and second-year undergraduate students are required to live in Andersen Hall. If you will be a freshman or sophomore and would like to be considered for off-campus housing, you must make a detailed request in writing to the Office of Student and Residence Life. If you are not approved to live off-campus and you fail to sign up for a room, you risk being billed for the room for the academic year."

Housing Forms and Due Dates

The following items should be submitted to the Office of Student and Residence Life as soon as possible to better ensure the fulfillment of your housing preferences. For optimal results, all housing forms, the emergency information form, and the $500 housing deposit should be received in the Office of Student and Residence Life by May 14, 2010. Applications and/or deposits received after May 14th and by June 30, 2010 will be placed in rooms as space allows, but are not guaranteed housing. Students who submit forms and/or deposits after June 30, 2010 will be placed on a waiting list and will be housed in Andersen Hall only as space becomes available.
