All students are required to comply with the immunization laws that are established by New York State.


Required Immunization Form (Due August 4, 2010)
  • Immunization History Form - New Students must submit a completed form to the Office of Admissions at Manhattan School of Music. This document will help students gather the immunity information as required by New York State health laws.

How Can I Obtain My Immunization Records?

Immunization records are typically obtained from following places:
  • Previous healthcare professionals (e.g. pediatrician, general practitioner, etc.)
  • Undergraduate institutions
  • Elementary or high school health care offices


How Do I Submit My Immunization Records to Manhattan School of Music?

After gathering immunization information, students may submit the Immunization History Form to the Office of Admissions by mail or by delivering it to the office in person. Records may also be faxed directly from a physician's office or a university, high school, or elementary school health office. The Office of Admissions fax number is 212-749-3025.

Why Do I Need To Submit My Immunization Records?

It is the law! New York State has immunization requirements for measles, mumps, rubella, and meningitis, which are presented in two health laws. These public health laws are listed below:
  • New York State Public Health Law 2165 requires that all students born on or after January 1, 1957 must provide the College with documentation of immunity to measles, mumps, and rubella.
  • New York State Public Health Law 2167, established July 22, 2003, requires academic institutions to distribute information pertaining to the meningococcal disease vaccination to all students. Furthermore, all students are required to submit a response form as to the status of the meningococcal disease.
