Admissions Documents Due
- Transcripts - Students must provide final, official,
and original transcripts from ALL previous institutions
attended. Note: Please send original official documents
only, copies and faxes are not considered official.
- Proof of Degree or Diploma - Official proof of
degree or diploma is required for all Freshman, Graduate,
and Doctoral students. Many U.S. transcripts will list a
date of graduation or a statement and date that a diploma/degree
has been earned.
- Immunization
History Form - To be completed, signed, and returned
by your physician or clinic personnel. This form must be
either faxed by your doctor/clinic with the office's cover
sheet or mailed by your doctor/clinic’s office.
New York State requires that all matriculated students show proof
of immunity to measles, mumps, rubella, and meningitis. Please review
the health information provided in the Disease and Vaccine Information
Statements, and use the Immunization History Form to ensure that
you are in compliance with all New York State health laws.
New York State health law requires that we provide you with information
about vaccination. Please visit the Centers for Disease Control
(CDC) Web site for the fact sheet on measles, mumps and rubella (MMR)
and the fact sheet on meningitis.

- Translations and WES Evaluations - International
students and any students who have studied abroad must provide
an official, notarized translation of a non-English transcript.
Any student who was asked by the Office of Admission to
provide a World Education Services (WES) Evaluation must
request that WES send an official and original report directly
to Manhattan School of Music. Note: Copies and faxes of
translations are not considered official.
